Job Summary

Applications are invited from suitably qualified candidates to fill the post of Learning Disability Community Nurse at the Mid-Atlantic Wellness Institute (Psychiatric Care Facility).

Job Description

Key responsibilities include:

  • Developing a range of meaningful activities in conjunction with the interdisciplinary team for Service Users to enjoy including training, leisure and employment opportunities and facilitate the Community Care Workers to implement these activities
  • Liaising with other members of community learning disability team to assess their needs, implement support plans and evaluate their effectiveness
  • Encouraging support staff to deliver person cantered care that enables Service Users to engage in meaningful activities and relationships
  • Acting as a case manager to develop programmes for designated individuals or groups of Service users
  • Providing 'hands on' skill training and job coaching for Service Users and staff
  • Ensuring completion of the Person Centred Plan (PCP) for each Service User and represent them at the PCP meetings, documenting all relevant information in the clinical notes


Qualifications &/ registration required:

  • Registered Nurse Mental Handicap (RNMH) or Registered Mental Health Nurse (RMN)
  • Certificate in Learning Disability Nursing or equivalent preferred 
  • Certification in CPR, Code Management, and NCI
  • Minimum of 3 years post-graduate experience
  • Minimum of 3 years experience working with learning disabled Service Users
