The incumbent conducts home visits and surveys to provide elderly care, support maternal and child health, and facilitate immunization while identifying environmental risks and referring clients to health services to ensure community well-being.

 

CORE FUNCTIONS

The successful candidates’ core functions will include: 

  • Conducting home visits 
  • Providing care for the elderly including meeting their hygiene needs/support elderly persons to meet their physical, emotional and psychosocial needs/ provide care and support for senior citizens
  • Identifying mobility needs and assist clients as required 
  • Observing, detecting and responding to signs of abuse 
  • Conducting periodic house to house survey of persons within the assigned area to obtain data on age groups and their health status, as directed
  • Referring clients with health problems as necessary to members of the health care team and other agencies 
  • Assisting Public Health Nurses & District Midwives in identifying all expectant and postnatal mothers in assigned area and ensuring that they attend clinic early and regularly to receive adequate prenatal and postnatal care respectively 
  • Assisting with the care of persons with mental and psychological disorders
  • Demonstrating knowledge of the stages of child development
  • Guiding parents and guardians to understand their roles and responsibilities 
  • Providing basic information and strategies in sexual health for men and women 
  • Facilitating the provision of information to clients (males and females) on sexual and reproductive health 
  • Encouraging regular attendance at Child Health Clinic to facilitate the immunization of children in a timely manner 
  • Assisting the Public Health Nurse in mass immunization programmes e.g. documentation and crowd control 
  • Identifying signs of environmental risk factors due to mosquito, rodent and other pests as well as improper solid waste management and refer to appropriate personnel 

 

QUALIFICATION & EXPERIENCE:

  • Two (2) ‘O’ Level/CXC subjects; English Language compulsory
  • Successfully completed a minimum of eight (8) weeks training approved by the Ministry of Health or HEART TRUST Certification in Community Health Aide/ Healthcare Assistant 

 

SPECIFIC KNOWLEDGE/SKILLS REQUIRED:

  • General ability to read and write well 
  • Excellent Customer Service Skills
