Under the supervision of the Multidisciplinary Mental Health Team, the Community Support Worker will be expected to provide ongoing support, daily care and assistance to persons with mental illness, to ensure that they achieve their maximum potential

 

Key responsibilities include:

  • Assists Service Users in organizing their daily living activities, including grocery shopping, meal planning, preparation and serving
  • Ensures ongoing training of the Service Users to achieve their maximum potential
  • Assists Service Users in organizing their social and recreational activities and also must actively engage in these activities with the Service User
  • Advocates for the Service Users; is an active member of the Multidisciplinary Team
  • Assists with the completion of Assessments and Individual Programme Plan (IPP) with all Service Users
  • Observes and monitors Service Users’ physical / mental state and reports any changes to the Charge Nurse / Resource Nurse
  • Ensures Service Users’ Programs are implemented, supports, assists and trains in varying areas as required
  • Ensures facilities are kept in good working order with minimal wear / tear on property. Reports damaged furniture as necessary
  • Other related CSW duties as assigned by the Charge Nurse, the Resource Nurse, the Clinical Supervisor or the Clinical Manager

 

Qualifications & Registration required:

  • Be registered with the Bermuda Nursing Council
  • Safety care Certificate and CPR would be an asset
  • Must have a current Bermuda’s driver’s license
