The Community Support Worker (CSW) will be expected to provide ongoing support, daily care and assistance to persons with mental illness, to ensure that they achieve their maximum potential.
The incumbent supports patient rehabilitation and daily living by assisting with personal care, facilitating social and therapeutic activities, monitoring physical and mental health, and ensuring patient safety during transport and community outings.
The incumbent provides comprehensive nursing care by assessing patient needs, developing and executing treatment plans, managing medication, assisting in emergencies, and educating patients, families, and staff to support recovery and discharge.
